For the safety and security of Ponte Vedra High School students and guests, we require that all attendees who are not Ponte Vedra High School students register with the Dean’s office.

ALL GUESTS NEED PHOTO IDENTIFICATION

• Guests must follow the Ponte Vedra High School Code of Conduct
• No guest will be admitted the night of the dance without this form being on file
• Once the guest or Ponte Vedra High School student leaves the event, they will not be allowed to return
• Guest must arrive and depart with the student
• Guest must be 20 years old or younger
