For the safety and security of Ponte Vedra High School students and guests, we require that all non-Ponte Vedra High School students attending register with the Dean’s office.

ALL GUESTS NEED PHOTO IDENTIFICATION

• Guests must follow the Ponte Vedra High School Code of Conduct
• No guest will be admitted the night of the dance without this form being on file
• Once the guest or Ponte Vedra High School student leaves the event, they will not be allowed to return
• Guest must arrive and depart with the student
• Guest must be 20 years old or younger
