ILLNESS GUIDELINES<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Deciding when to keep your child home from school can be difficult. When a child is sick and needs to stay at home, parents should contact the school and describe the illness and symptoms. If a medical provider makes a specific diagnosis (such as strep throat, conjunctivitis, chicken pox, etc.), let school staff know.<\/p>

A parent\/guardian will be called whenever a student has a fever over 100.4 degrees, diarrhea or vomiting, significant pain, repeated complaints, or anything out of the\u00a0ordinary.<\/p>

Please remember that if students are sent home with a fever, they must be fever free for 24 hours before they return. This means that they should not return the next day. The 24 hour period begins from the time they no longer have a fever.\u00a0<\/p>

There are several reasons to keep (exclude) sick children from school:<\/b><\/p>

1. The child does not feel well enough to participate comfortably in usual activities, such as with extreme signs of tiredness or fatigue, unexplained irritability or persistent crying.<\/p>

2. The child requires more care than the school staff is able to provide without affecting the health and safety of the other children.<\/p>

3. The illness is on the list of symptoms or illness for which exclusion is recommended.<\/p>

4. The child is not vaccinated due to medical or religious reasons and there is an outbreak in the school.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Please complete the appropriate medication form(s)  (see links below) for your student to have medications at PONTE VEDRA HIGH SCHOOL. <\/p>

If your student requires prescription medication<\/strong> to be kept in the school clinic, this form must be completed and signed by the physician and the parent\/guardian:   Rx Medication Authorization Form<\/a><\/p>\n

(Note: ALL prescription medications must be kept in the clinic, with the exception of inhalers, EpiPen\/Auvi-Q auto-injectors and emergency Glucagon.)  Students found in possession of medications without the proper paperwork on file will be subject to disciplinary action as noted in the Student Code of Conduct.<\/p>\n

Students may self-carry over-the-counter<\/strong> medications by submitting this completed form to the clinic and keeping a copy with them at all times:

All OTC medications must<\/strong> be in the original packaging (NO loose pills). Student\/parent may also choose to keep their OTC medication in the clinic.<\/p>\n

Note: We must have new<\/strong> medication forms on file each school year<\/strong>.<\/p>\n
